Finance Forms

Print the following forms to make authorized purchases on behalf of PTA,
gain reimbursement for expenses, or present monies for deposit:

Texas Sales & Use Tax Exemption-
PTA is exempt from payment of sales and use taxes. This certificate should be presented to the supplier when a member makes an authorized PTA purchase.

PTA Disbursement Voucher -
Members are reimbursed for all authorized PTA expenses. Complete this form, staple original receipts to the top center, and place it in the box marked Treasurer in the school office.

Itemized Receipt Form  -
This form must accompany any checks or cash presented to the Treasurer for deposit.
