2010-2011 STUDENT INFORMATION FORM

PLEASE READ THESE INSTRUCTIONS CAREFULLY:

• Complete ONE FORM PER FAMILY (see below). 
• Fill out ONLY that information that you would like included in the directory.
Leave blank any areas you do not want published.
• Your signature grants PTA permission to publish information provided on this form.
• Parents have the opportunity to deny access to directory information using the FERPA and Texas Public Information Act signature page located in the NISD student-parent handbook, which is due to the school within the first two weeks of enrollment.
• If we do not receive THIS form from you, we will use information from the FERPA Parent Information Release Authorization form on file with NISD. Directory information is considered public information under the Texas Open Records Act, unless an assertion of privacy is made by the person concerned.
